Transitions Program Supervisor

Purpose of Position: The Transitions Program Supervisor is responsible for the scheduling and supervision of support staff operations across the Transitional Housing Program. This position provides back-up case management support and will be the subject matter expert for our electronic data entry programs. 

Supervisory Responsibilities: As a member of the Transitions Program Department, the Transitions Program Supervisor will oversee a team of approximately seven support staff who are located across two campuses in Eugene, Oregon. This position will conduct routine 1:1s with their direct reports to ensure team members are provided with direction, support and accountability.

Case Management Responsibilities: This position will manage a small caseload of clients at our Roosevelt Campus. Individualized case management is a core component of Sponsors’ programs, and case managers serve a vital role in conducting assessments, initiating program referrals, developing dynamic case plans, and in carrying out the overall operations of the Transitions Program.

Administrative Responsibilities: The Transitions Program Supervisor will serve as the subject matter expert for our electronic data entry systems. This position will conduct routine quality assurance audits to ensure integrity of our client documentation standards. 

Position Reporting:  This position reports directly to the Transitions Program Director.

Job Responsibilities:

Assist in the coordination of program operations.
• Provide direct supervision to resident night managers, resident weekend managers and support staff team members.
• Conduct routine 1:1s with direct reports.
• Conduct routine quality assurance audits as assigned by the Transitions Program Director.
• Serve as subject matter expert for Service Point, CDL, TITAN and other electronic systems.
• Assist the Transitions Director with oversight of the program application process.
• Monitor common areas and individual housing units to ensure the safety, security, and cleanliness of the facility.
• Receive and document physical donations (e.g., clothing items)
• Receive and route calls, faxes, and walk-in program visitors.

Oversee all aspects of Case Management for assigned program participants.
• Perform intake for new residents within three business days of arrival using program-approved assessment tools.
• Monitor status of individualized case plans based on program assessment tool recommendations.
• Initiate referrals to community partners and communicate regularly with participants’ parole officers and treatment providers.
• Monitor compliance with Sponsors program requirements.
• Monitor compliance with requirements of other related programs, including cognitive groups, drug/alcohol treatment, parenting classes, and others as applicable.
• Conduct weekly scheduled 1:1 sessions with each participant and maintain communication as necessary.
• Perform scheduled and random drug and alcohol screenings.
• Utilize web-based programs and other systems to chart participant progress, maintain accurate entry/discharge records, and coordinate care.
• Facilitate Sponsors cognitive groups and ensure program fidelity, participant compliance, and maintain all group related paperwork in the agency designated time frame.

Ensure program participants are compliant and progressing appropriately in transition/exit planning
• Assist participants in developing transition plans with a goal toward long-term stable housing that meets the approval of assigned parole officers.
• Monitor and discuss participants’ employment, savings, and financial management, particularly as it relates to their transition from Sponsors and into stable housing.
• Maintain appropriate participant records, both electronic and physical, as required by Sponsors, collaborating agencies, and grant requirements.

Intra-Agency Collaboration and Communication
• Attend and actively participate in scheduled program staff meetings, monthly all-staff meetings, and weekly case management meetings.
• Participate in scheduled trainings as requested by management staff.
• Maintain regular direct communication, both written and verbal, with program supervisors and other Sponsors staff regarding participant progress and conduct, facilities operations, and organizational protocols/procedures.

Education and Experience:
• 1+ year of experience providing direct client services in social service setting
• Experience with electronic health record systems
• 1+ year of supervisory experience preferred
• Higher education degree in related field preferred

Work Environment: The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties.  This position will travel between locations at the direction of the supervisor in an effort to meet organizational needs. May encounter frequent interruptions throughout the work day.

Physical Demands: Walking upstairs, sitting at a desk for prolong periods of time. Noise level in the work environment is typical of that of an office environment.
