Roosevelt Case Manager

Purpose of Position: Support the mission of Sponsors assisting men from Lane County released from Oregon state correctional facilities and the Lane County Jail in making a successful re-entry into our community.  Sponsors is nationally recognized as a model utilizing best practices in re-entry services.

Reports to:  Director of Case Management

Supervisory Responsibilities: No direct supervisory responsibilities. May provide oversight and mentorship to interns and training components to new hires.

Primary Objective: Individualized case management is a core component of Sponsors’ programs, and case managers serve a vital role in conducting assessments, initiating program referrals, developing dynamic case plans, and in carrying out the overall operations of the Transitional Housing program. 

CASE MANAGER RESPONSIBILITIES:

Oversee all aspects of Case Management for assigned program participants.

  • Perform screening and intake for incoming residents using program-approved assessment tools.
  • Monitor status of individualized case plans based on LS/CMI transitional assessment and/or reassessment as needed.
  • Coordinate and communicate regularly with participants’ parole officers and treatment providers.
  • Monitor compliance with conditions of each participant’s release and all Sponsors program requirements.
  • Monitor compliance with requirements of other related programs, including cognitive groups, drug/alcohol treatment, parenting classes, and others as applicable.
  • Conduct regularly scheduled 1:1 session with each participant and maintain communication as necessary.
  • Perform scheduled and random drug and alcohol screenings.
  • Utilize web-based programs and other systems to chart participant progress, maintain accurate entry/discharge records, and coordinate care.
  • Ensure program participants are compliant and progressing appropriately in transition/exit planning
  • Assist participants in developing transition plans with a goal toward long-term stable housing that meets the approval of assigned parole officers.
  • Monitor and discuss participants’ employment, savings, and financial management, particularly as it relates to their transition from Sponsors and into stable housing.
  • Maintain appropriate participant records, both electronic and physical, as required by Sponsors, collaborating agencies, and grant requirements.

Assist in the coordination of program operations.

  • Monitor common areas and individual housing units to ensure the safety, security, and cleanliness of the facility.
  • Receive and document physical donations (e.g., clothing items)
  • Receive and route calls, faxes, and walk-in program visitors.

Intra-Agency Collaboration and Communication

  • Attend and actively participate in scheduled program staff meetings, biweekly all-staff meetings, and weekly case management meetings.
  • Participate in scheduled trainings as requested by management staff.
  • Maintain regular direct communication, both written and verbal, with program supervisors and other Sponsors staff regarding participant progress and conduct, facilities operations, and organizational protocols/procedures.

Skills and Knowledge:

  • Excellent verbal and written communication skills.
  • Ability to foster teamwork, promoting process improvement and build relationships
  • Proficient with Microsoft Office Suite or related software
  • Ability to work effectively with a wide range of people to achieve individual and organizational objectives
  • Committed to the mission and values of Sponsors and reentry services

Education and Experience:

  • Bachelor’s degree, in related field preferred
  • Corrections or reentry experience

Work Environment:

The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties.  May encounter frequent interruptions throughout the work day.

Physical Demands: 

Walking upstairs, sitting at a desk for prolong periods of time. Noise level in the work environment is typical of that of an office environment.
